Transitioning to our Learning Disability Services

We provide health input to adults with learning disabilities in Hampshire and Southampton who have identified unmet health needs that cannot be solely addressed through mainstream services. 

We work with the individual, their families, carers and other supporting services on their referred need, to achieve their health goals, after which, they will be discharged from our service. Individuals can access our service more than once when our Learning Disability Service criteria are met. 

Whilst referrals can be made into the team six months before the individual’s 18th birthday, we cannot begin specific assessments and interventions until after their 18th birthday.  Time prior to this can be used to support the individual’s transition into our service, through information gathering and attending transition meetings - for example, to help work out the support required from our team, once the person turns 18 years old.   

Individuals can self-refer or referrals can be made on the individual’s behalf, provided it is with their consent or made in their best interest (in the absence of their ability to consent).

  • All transition referrals will go through the Single Point of Access Team for assessment. 

  • Referrals should be made using our referral form with the young person's consent, or with reference to their lack of capacity to consent and best interest considerations.

Do you know that people with learning disabilities can have an annual health check from the age of 14 and over? 

The GP surgery should send out yearly invitations, however the GP surgery should be contacted if this has not been received.

  • The Panel has been established to bring together a multi-agency group of service and operational leads from education, health and social care. Their role is to provide leadership on transition processes, overseeing and monitoring the transition of specific cases where a more focused, managed multi-agency approach is required.
