We provide health input to adults with learning disabilities accross the areas of Hampshire, Southampton, Portsmouth and the Isle of Wight who have identified unmet health needs that cannot be solely addressed through mainstream services.
We work with the individual, their families, carers and other supporting services on their referred need, to achieve their health goals, after which, they will be discharged from our service. Individuals can access our service more than once when our Learning Disability Service criteria are met.
Whilst referrals can be made into the team six months before the individual’s 18th birthday, we cannot begin specific assessments and interventions until after their 18th birthday. Time prior to this can be used to support the individual’s transition into our service, through information gathering and attending transition meetings - for example, to help work out the support required from our team, once the person turns 18 years old.
Individuals can self-refer or referrals can be made on the individual’s behalf, provided it is with their consent or made in their best interest (in the absence of their ability to consent).
The Portsmouth Learning disability healthcare services are integrated with Portsmouth City Council, therefore the process is slightly different.
The integrated learning disability service (ILDS) in Portsmouth accepts referrals for young people with a learning disability from the age of 14 upwards. This is with the aim to provide an efficient and timely triage.

- To provide a positive experience of first contact with ILDS
- Reduce risk, discrimination, missed opportunities for positive intervention and essential sign posting for those who are not eligible for ILDS or have needs just above the threshold
- Young People with SEND are well supported to prepare for and transition to adult life to achieve the best possible outcomes in health, independent living, community participation and employment.
However, healthcare support from the team will not commence until a young person turns 18.

All transition referrals will go through the Single Point of Access Team for assessment.

Portsmouth
Referrals to the teams will be accepted (from any source) in relation to adults (aged 18 or over) with a learning disability and a healthcare need, which together create problems in the provision of care, treatment and/or management. The support required must go beyond that which is reasonably available from generic services.
Referrals can be made through SystmOne or by completing a referral form. Forms can be requested by contacting the Kestrel Centre on 0300 123 4019 or via email at hiowh.portsmouthldhealthadmin@nhs.net
Isle of Wight
You can make a referral to the team by:
- Using the Community Learning Disability Referral form.
- Self referral by service users
- Referrals from GPs or other health professionals, Local Authority, or from families and carers preferably using the Referral form above and via email into the Learning Disabilities mailbox:iownt.DisabilitiesAdmin.Learning@nhs.net
If it is not clear that the person being referred will meet our criteria, then we suggest completing our screening tool and send that with the referral to save time in this having to then be sent out for completion.

Do you know that people with a learning disability can have an annual health check from the age of 14 and over?
The GP surgery should send out yearly invitations, however to be invited you must be on the GP surgeries learning disability register. If your not sure if you are on the register please ask at your surgery.

The Panel has been established to bring together a multi-agency group of service and operational leads from education, health and social care. Their role is to provide leadership on transition processes, overseeing and monitoring the transition of specific cases where a more focused, managed multi-agency approach is required.
