Our Learning Disability Nurses work in our Community Learning Disability teams based across Hampshire, Southampton, Portsmouth City and the Isle of Wight.
Our nurses are part of multi-disciplinary teams who work with occupational therapists, speech and language therapists, psychologists, art therapists, psychiatrists, and physiotherapists to provide the best possible coordinated care for people with learning disabilities.
The nurses work with people who have a learning disability to ensure their health needs are identified and appropriately met, including facilitating access where appropriate to mainstream services. They support in a variety of areas to include areas such as Dementia and functional change, Mental Health, Epilepsy, Complex health and multiple physical health conditions as well has behaviours that challenge.
Our nurses work with a variety of people and services to include GP’s in their surgeries, in some areas they work in hospitals providing hospital liaison, with Adult Services and with other health and social care providers to ensure that reasonable adjustments are in place to reduce inequalities in healthcare.
