Our award winning specialist team of Diabetes Nurses, Dietitians and Consultants work alongside people diagnosed with diabetes across West and North Hampshire. They aim to provide holistic care, supporting people who have been diagnosed to self-manage (where possible) as well as linking with other local specialist services such as Talking Therapies, GP surgeries and community care teams. Dr Kate Fayers | Consultant Diabetologist
Qualifications: BSc in Biochemistry, MBBS, MRCP, FRCP
- I developed an early interest for Quality Improvement supporting the development of Think Glucose, a national diabetes inpatient project.
- Working with colleagues, I have managed a wide range of service improvement projects including the roll out of WISDOM, a blueprint for PCN diabetes care recognised by the Quality in Care Diabetes Awards 2020.
- Community led specialist diabetes care is at the heart of my strategy to improve patient experience.
Dr Hermione Price | Consultant Diabetologist
Qualifications: BSc Hons, MBChB Hons, DPhil, FRCP
- I offer consultant clinics and participate in education delivery for people with diabetes.
- I am the lead for diabetes research for Southern Health foundation trust, the joint lead for diabetes at the Wessex clinical research network and an Honorary senior lecturer at the University of Southampton
- I am a supervisor for junior doctors in training and PhD students.
- I am the Trainee Programme Director for Diabetes and Endocrinology in Wessex
Dr Ben Houlford | Consultant Diabetologist
Qualifications: BSc (Hons), MSc, MB BS, MRCP (UK)
- In my clinic I mostly see people with a new diagnosis of type 1 diabetes
- I also work at Lymington New Forest Hospital as a Consultant in Diabetes & Endocrinology and General Medicine, including running the inpatient diabetes service and looking after medical inpatients
- I am named clinical supervisor for junior doctors and mentor for a trainee advanced nurse practitioner
- I am the Associate Clinical Sub Dean for Lymington New Forest Hospital.
Dr Sarah Brewster | Consultant Diabetologist
Qualifications: BSc (Hons), MBBS, DPhil, FRCP
- I offer consultant clinics and am a named clinical supervisor for junior doctors
- I am the Trust lead for Monogenic Diabetes
- I completed a PhD looking at the role of community pharmacy in supporting those living with diabetes, particularly those who may be most in need, and have an interest in how we can best support these individuals.
Dr Clare Whicher | Consultant Diabetologist
Qualifications: MBBS with distinction, MRCP, PhD
- I work with the clinical team to support people living with diabetes. I am passionate about diabetes care in vulnerable or often forgotten patient groups, including people with co-existent mental health issues and preconception planning.
- I have undertaken a PhD looking at diabetes, weight and mental health.
- I am a former chair (2019-2020) of the Young Diabetes and Endocrinologists (YDEF) committee.
Caroline Atkinson | Lead Diabetes Specialist Nurse
Qualifications: Registered General Nurse, Non-medical Prescriber, Diploma in Diabetes Care
- I was appointed as Lead Diabetes Specialist Nurse when the West Hampshire Community Diabetes service was launched. We see people with diabetes in clinics and provide educational sessions to people with diabetes, health care professionals and carers.
- I support people with managing their diabetes, managing the helpline and clinical appointments.
- I was instrumental in supporting to transform the service to a virtual platform during the COVID pandemic.
Maggie McDonald | Senior Diabetes Specialist Nurse
- I lead on the provision of Type 1 diabetes education, my current focus is on making education and resources available online. I have produced and featured in various new Diabetes educational video resources and education courses.
- I offer diabetes clinics and undertake patient triage.
- I am involved in operating the diabetes helpline, offering urgent advice to patients with diabetes
Emily Perry | Diabetes Specialist Nurse
- I offer nurse-led clinics to patients with type 1 and type 2 and also help to run the patient helpline.
- I support type 1 diabetes education
- I deliver ‘Cluster’ meetings for GP practices across West Hampshire, which offer an opportunity for sharing best practice and updates in diabetes and education.
- I am currently involved in a Primary Care Network Transformation project which involves running co-clinics with primary care clinicians for more complex diabetes patients
Milly Pickburn | Diabetes Specialist Nurse
Qualifications: Advanced Diploma in Child Nursing, CDEP, BERTIE, Certificate in Optimising glucose control in Type 2 Diabetes Mellitus and Certificate in Diabetes Care (CIDC)
- I work for the diabetes team, delivering clinics and telephone reviews for patients with diabetes. I also spend time on the diabetes helpline speaking to patients with diabetes about any urgent queries and offering advice.
- I also contribute to the delivery of education for patients as well as education for health care professionals on management of diabetes.
Margaret Stubbs | Diabetes Specialist Nurse in North Hampshire
Qualifications: MSc in Primary Care 2006, Pos Grad Dip in Diabetes 2014, Independent Prescriber 2004
- I work within a multidisciplinary team to support patients with diabetes, particularly those who find it hard to access routine diabetes care.
- I work with patients with mental health issues, learning disabilities or other care needs who find it hard managing their diabetes without support. This includes education of carers for patients with diabetes, as well as education of health care professionals.
Sherri Johnston | Diabetes Specialist Nurse in North Hampshire
Qualifications: Registered General Nurse, BSc Honors, Independent Non-medical Prescriber, Diploma in Diabetes Care.
- I work with primary care to provide support and education for staff and patients, with the goal of delivering high quality diabetes care throughout North Hants.
- We encourage regular T3 meetings (Treatment to target) where clinic notes are reviewed and audited. This includes reviewing patients who have had complications of cardiovascular or peripheral vascular disease, amputations or acute diabetes complications such as hypoglycaemia or DKA with the aim of improving diabetes care and treatment going forwards.
Jan Mitchell | Diabetes Specialist Nurse
Qualifications: BA in post compulsory Education
- I work as a diabetes nurse offering clinics to patients with type 1 and type 2 diabetes, as well as answering advice queries and reviewing patients over the phone.
- One of my main interests is healthcare professional education, looking at improving diabetes care delivery in primary care as well as educating carers too. I run an insulin administration programme for carers.
Brian Lomax | Diabetes Specialist Nurse
- I work with patients with type 1 and type 2 diabetes. My main focus is on patient contact, engaging in clinics, setting goals with patients and supporting them to achieve them.
- I also enjoy working with health care professionals in primary care to improve diabetes services, using knowledge I gained from 20 years working as a district nurse and community matron.
Sue Cradock | Diabetes Specialist Nurse
Qualifications: PhD RGN DipN FETC NMP
- As well as clinical roles, I work to re-design and deliver professional education programmes, in collaboration with the LEaD team.
- I have been working with community teams, including engaging and training non registered practitioners and paid carers, in insulin administration.
- I also work with two primary care groups looking at non-pharmaceutical diabetes interventions.

- I work within the Diabetes team as a DSN.
Kelly Carden | Diabetes Specialist Nurse
Qualifcations: BN Adult nursing, Certificate in Optimising glucose control in Type 2 Diabetes Mellitus University of Warwick
- My current focus is on Type 1 diabetes education and technology education for patients, particularly around the Freestyle Libre and Dexcom.
- I also have a special interest in Diabetes and mental health, as well as looking at transition services for young adults.
- I offer diabetes clinics to patients, as well as helping with triage and operating the diabetes helpline.
Sally Penny | Diabetes Specialist Nurse
Qualifications: Registered General Nurse
- Previously worked in primary care settings and joined the DSN team in 2019.
- I work with patients who have type 1 or type 2 diabetes, supporting them in clinic or telephone appointments. I also answer helpline queries.
- Currently working on a project to support nurses in primary care managing patients with diabetes.
Fionnagh Roberts | Diabetes Specialist Nurse
Qualifications: Diploma of Higher Education in Adult Nursing, Certificate in Diabetes Care from Warwick University, Independent Nurse Prescriber
- I work as a Diabetes Specialist Nurse one day a week. I am currently involved in a transformation project supporting primary care and upskilling those working in GP surgeries in their management of people with diabetes.
- I also co-deliver our SWITCH course, which educates those working in primary care to initiate insulin and other injectables.
Avril Smith | Diabetes Specialist Nurse
Qualifications: RGN BSc (Hons), Bradford Course Diabetes Management with Advice, PGdip Diabetes, Independent Non-Medical Prescriber
- I returned to join West Hampshire Community Diabetes Team in June 2023 with a temporary contract for one year to cover absence due to the PCN project
- I also work in General Practice as an ANP and lead for diabetes management within the practice, initiating treatments, on-going education and supporting the other HCP and GPs.
- Good patient care and fairness to all is my philosophy for work.
Elaine Turberville | Diabetes Specialist Nurse
- I previously worked as a Practice Nurse with a special interest in diabetes. I joined the DSN team in 2023.
- My main role is supporting and empowering patients with their diabetes care.
- I am involved in diabetes clinics, the diabetes helpline, and offering urgent advice to patients with diabetes.
Georgette Alayyan | Lead Diabetes Specialist Dietitian
Qualifications: UK Registered Dietitian (R.D.) BSc Nutrition and Dietetics, regulated by the HCPC (DTN 27339) and a member of the British Dietetic Association (BDA).
- I lead the education work stream which involves delivering T1 education, running workshops on carbohydrates and insulin, producing a series of online educational video resources, providing training to other health care professionals and delivering content through our social media channels
- I provide specialist dietetic advice and developed our T2 remission pilot ‘SHAKE’
- I’m the South East representative for the BDA diabetes specialist group.
Pepa Richardson | Diabetes Specialist Dietitian
Qualifications: MSc Dietetics and Nutrition, UK Registered Dietitian (HCPC number DT031679). Member of the British Dietetic Association (BDA) and the BDA diabetes specialist group and Professional Member of Diabetes UK.
- I provide specialist dietetic advice for patient living with diabetes
- I deliver educational workshops to help those living with diabetes manage their condition
Chloe Wheeler | Diabetes Specialist Dietitian
Qualifications: UK registered dietitian, BSc dietetics and nutrition, regulated by the HCPC and a member of the BDA
- I support those living with diabetes in a dietitian-led clinic, offering specialist advice, mainly around carb counting.
- I am involved in the delivery of our education sessions, as well as being heavily involved in the production and filming of our education videos.
Lisa Whale | Diabetes Educator
- I help deliver courses for people with type 2 diabetes, both newly-diagnosed patients and those who have had type 2 diabetes for many years.
Tom Pratt | Diabetes Educator
- I also help deliver courses for people with type 2 diabetes.
James Irwin | Administration Manager for Diabetes and Respiratory
- I work closely with the admin teams and clinical teams across both the Diabetes and Respiratory services within the West, Mid and North Hampshire teams.
Alli White | Personal Assistant to Dr Price, Dr Alsafadi and Dr Fayers
- I am involved in the organisation of clinics for both new and existing patients
- I process new referrals and organise the diabetes team triaging system
- I organise the work diaries for members of the diabetes team
- I organise patient investigations and process investigation results
- I process clinic letters and investigation results and post these to our patients.
Louise Westbury | Clinical Diabetes Improvement Champion for North and Mid Hants, and MDT Coordinator for West Hants
Qualifications: I am currently undertaking a level 3 diploma in Youth Work practice
- Organising and supporting cluster meetings for West and Mid Hants
- Presenting data to North Hants alongside T3 meetings
- Collating and crunching data reports for GP practices in all areas
- Running diabetes reports and sharing data
- General administration roles.
Donna Hibberd | Educational Co-ordinator Lead for Diabetes
- I am the education co-ordinator lead for diabetes, which involves organising education on Freestyle Libre, type 1 diabetes, carb counting and carb & insulin education courses and the DIAMOND education course for new type 1 patients.
- I also work as a line manager for the administration team and carry out administrative functions.
- I am involved in various education projects.
Claire Bartlett | Administrator - North and Mid Hants Community Diabetes Service
Qualifications: English Joint History Degree and PGCE in English
- I oversee all admin within the North team
- I also organise North events
