About the service

Our awarding winning specialist team of diabetes nurses, dietitians, educators and admin team support people living with diabetes across South East Hampshire. We aim to provide care, supporting people who have been diagnosed to self-manage (where possible) and we link with other local specialist services such as Talking Therapies, doctors surgeries and community care teams

What we do

As a non case-holding team we do not receive referrals but we receive requests for clinical guidance and education.

We run courses for people with Type 2 Diabetes across the South East of Hampshire. These courses can help you to learn more about your condition and manage it more effectively. It’s also a great opportunity to meet other people with diabetes and to ask questions or voice concerns. Take a look at some of our courses below.

South East Hampshire Community Diabetes Service are running online group education sessions for people living with type 2 diabetes. The New2Diabetes session are designed to provide you with up-to-date information and advice to help you manage your diabetes.

Our education offering consists of watching a series of short education videos in your own time, plus a 90 minute, interactive, online group Zoom session. To include:

  • A recap on what type 2 diabetes is
  • Living with diabetes; how sleep, stress, activity and food can affect your blood sugars and easy ways of managing them.
  • How to look after your feet
  • An open forum for your diabetes questions
  • Eating well with diabetes and updates on dietary advice
  • Tips from psychology to help you change your habits and deal with cravings.

The New2Diabetes course for those newly diagnosed with type 2 diabetes run online every Wednesday morning from 9.30am to 11.00am

We provide a four hour course aimed at people with Type 2 diabetes who have either been diagnosed for longer that 12 months or feel they need fresh input into managing their condition.

Education for Healthcare Professionals

The South East Hampshire and West Hampshire Community teams together with the LEaD training department also offer diabetes care and education to healthcare professionals across Hampshire at a variety of locations. We offer a varied portfolio of education for patients/ service users and healthcare practitioners. Wherever possible we aim to support patients/ service users to improve self care. For staff not employed by Southern Health there may be a cost associated with some courses. External applicants will have to complete an external application form.

We provide a three hour basic introduction to diabetes to staff to help them  support, manage and care for their residents with diabetes.

IMIT is aimed at health care professionals, GPs and nurses who have already completed some diabetes training and are going to be managing and initiating insulin and other injectable therapies.
