Learning Disability Awareness Week 2024 took place on 17 - 23 June 2024. The theme of the week was 'Do you see me?'

Learning Disability week is an opportunity to shine a light on the needs of people with learning disabilities, improve general awareness of what good health care looks like and celebrate individual achievements.

Lots of GP practices across Hampshire and Isle of Wight (including Southampton and Portsmouth) have now received their Sensory and Communication boxes. Look out for the posters your GP reception area.

Our Strategic Health Facilitators worked with our local Primary Care Coordination Network colleagues and developed an Easy Read GP Practice Registration Form and an Easy Read Family History form. 

These forms can be used as a template and amended by each GP practice or Primary Care Network (PCN) as needed.

These forms were reviewed by our Service User group to ensure that they were user friendly.

People with a Learning Disability aged 14 and over can have a free Annual Health Check, please contact your GP Surgery to arrange an appointment.

Our service works with people over 18 years of age.

Feedback form 

Our newly designed easy read annual health check feedback form can be downloaded, amended and used by patients, carers and GP practices. The information can then be used to support patients in a person-centred way and inform practice service development. If you are a clinician undertaking annual health checks, you can download the best practice checklist here.

The Learning Disability Friendly accreditation is now available to all GP Practices in Hampshire and Isle of Wight (including Southampton and Portsmouth).
