About the service
The Urgent Response Service is a multi-professional team who treat adult patients (18+) with a focus on avoiding hospital admission and facilitating early discharge from hospital. We have two teams, one who support people in their homes during a health crisis and one based in the Emergency Department at University Hospital Southampton (URS in ED).
What we do
The Urgent Response Service utilises a multi-professional team who treat adult patients (18+) , with a focus on avoiding hospital admission and facilitating early discharge from hospital.
Our community team have the ability to provide a 2 hour response time whereby the primary aim is to stabilise the patient at home providing skilled nursing care, therapy interventions, and care support. Length of stay on our caseload is, on average, up to 7 days.
Examples of intervention for patients on our community caseload are:
- Clinical Monitoring
- Medication administration (including IV antibiotics)
- ECGs
- Equipment provision
- Urgent Physiotherapy/Occupational Therapy
- Personal care
- Diet and Fluid support
The Emergency Department Team is also a multi-professional team. Working within the hospital this team provide a number of services including:
- Assessing individuals
- Safe, co-ordinated discharge planning
- Transition of care between the hospital and community services
- Involvement of relatives and carers in discussion
- Treated with dignity, compassion and all consideration given to cultural and religious diversity
- Providing high standards of care
Accessing the service
To refer a patient in the community to our service, please telephone on the number above. The patient’s details will be taken and you will then be transferred to a Shift Coordinator who will discuss the clinical crisis details and triage the referral for suitability / capacity.
Please note: Referrals must come from a health and social care professional and is only available to adults who are registered with a Southampton City GP.
The Emergency Department Team accept referrals made by Health and Social Care Professionals based at University Hospital Southampton. Referrals are made by telephoning the team and face to face contact. The Emergency Department Team can only assess patients currently attending the Emergency Department.
