About the service
The Transition Therapy Team offers occupational therapy and physiotherapy input to young people, between the ages of 14 - 25 who have complex physical disabilities. These disabilities are predominantly neurological, and some young people also have learning disabilities.
What we do
We provide occupational therapy and physiotherapy input to young people between the ages of 14 - 25 who have complex physical disabilities. These disabilities are predominantly neurological, and some young people also have learning disabilities.
We work alongside young people and their family or carers to support growing independence, helping young people to feel more able to take the lead in their health care.
Our team works with people to support them to develop a wide range of skills as well as linking with other agencies to find opportunities for the future.
Accessing the service
To access the service you must be aged between 14 - 25 years with complex physical disability and be registered with a Southampton GP.
