Stoma Care Service

About the service

The stoma care service will provide education, informal counselling, lifestyle advice, health promotion, appliance management and support to clients who have or are about to have a stoma formed.  The service involves both pre-operative and long term management of clients with a stoma. The stoma care service consists of a team of specialist nurses with a stoma care qualification. They assess clients and recommend appliances and accessory stoma products to be prescribed by the GP.
 

What we do

The stoma care service has the following objectives;

  • To provide specialist assessment and ongoing management from a stoma care specialist with a qualification in stoma care, 
  • To provide pre-operative and post-operative support to clients undergoing urological or colorectal surgery and stoma formation, 
  • To ensure clients are assessed by a stoma care specialist and prescribed appropriate stoma products, 
  • To ensure clients are prescribed and receive stoma care products, 
  • To provide a basic level of training to health care professional within the local acute trusts and primary care who may care for clients with a stoma, 
  • To support and react to relevant local and national support groups.  To review stoma care products and trial new products to ensure best practice, 
  • To continually monitor quality of care through clinical support. 

Stoma care clinics are held at the Bitterne Health Centre, Adelaide Healthcentre, Hythe Hospital, and Lymington Hospital by contacting the service for an appointment.

Accessing the service

The service works with all ages in Southampton City and South West areas of Hampshire. 

Referrals are taken from all healthcare professionals including GPs, clients can also self refer. 

A specialist nurse from the stoma care service will liaise with the colorectal specialist nurses when clients with colorectal cancer are referred to the service. 

Clients with non-malignant conditions are referred to the service by a colorectal consultant generally via a referral letter. Urology clients for cystectomy and ileal conduit are referred via fax following completion of a stoma care referral form by the urology specialist.
