About the service
We are a multi-disciplinary team of specialist mental health nurses, consultants, a social worker, health care support workers, a psychologist and occupational therapists. We work with older adults suffering with mental illness requiring assessment of memory and possible diagnosis treatment and monitoring.
What we do
We provide assessment, care and treatment of older adults living in the community and residential care homes who are experiencing a range of mental health issues. We will assess people for diagnosis and treatment of dementia and assessment and treatment for those suffering with other mental health disorders.
Patients will be seen in either clinic or at home, where most appropriate. Various treatment options and recommendations will be made depending on the outcome of the assessment and discussed in conjunction with the consultant. Treatment options will be discussed if this is appropriate for your loved one. We will also provide information, refer for ongoing support or discharge from our service where appropriate.
Accessing the service
Access to the OPMH community services is via referral from healthcare professionals only. These may be from primary or secondary care services or 111.
Our target is to see patients within 6 weeks, or sooner, depending on needs.
To make a referral, please complete a referral form on SystmOne including: desired outcome, patient goals, presenting situation, brief history, how long has the issue been going on, impact on life, risk, ability to attend clinic or home visit, dementia screening bloods, memory testing, consent to referral and NOK details.
