About the service
The Community Stroke Team offers a specialist service to people over the age of 18 who have had a stroke.
What we do
We work with patients, family and carers to support independence and improve quality of life for patients who have had a stroke.
Our team supports patients in the community and works with them to achieve their individual rehabilitation goals.
The team consists of occupational therapists, physiotherapists, nurses, speech and language therapists and psychologists.
Accessing the service
Access to the service is via referral only.
To access the service you must be must be registered with a GP in Southampton or the surrounding area.
