Community Neurological Services and Rehabilitation

About the service

Our Community Neurological Service specialists in the treatment and rehabilitation of people with neurological conditions such as Multiple Sclerosis, Parkinson's Disease, Motor Neurone Disease, Head Injury, Cerebral Palsy and Stroke. The teams consists of occupational therapists, physiotherapists, rehabilitation assistants, doctors, dietitians, speech & language therapists and they work alongside existing services within the health and social care system.

Following assessment in the home, the team will decide which treatments are most appropriate. Services are provided within Portsmouth and Southampton.

The team also works closely with the Community Neurological Rehabilitation Gym. Based at Adelaide Health Centre in Southampton, this service uses Physiotherapists with a special interest in neurology and Rehabilitation Assistants to provide individual neurological assessment and treatment sessions and self management for people living with long term neurological conditions.

What we do

The Community Neurological Service is a specialist multi professional service for adults with neurological conditions such as Multiple Sclerosis, Motor Neurone Disease, Parkinson’s Disease and Traumatic Brain Injury.  The service is intended to work alongside existing services across the health and social care system.

The service aims to provide access to a range of co-ordinated, multi professional interventions to enable patients with neurological conditions to retain optimal independence within their home environment. The team will also proactively support the changing needs of patients and promote self management within the community, alongside expanding expertise in the community management of neurological conditions, providing a resource for inter-professional support and education.

Once allocated to the service, patients have access to assessment, intervention and review of their ongoing and changing needs. When indicated, direct, co-ordinated multi professional intervention is provided for patients with complex needs, with signposting to other services as indicated. The service will provide case management with advice and education of patients and carers and promotion of self management. Support will be provided to professionals providing ‘usual services’ to improve the quality of care across community services for patients with neurological conditions.

Accessing the service

Southampton patients
Patients must be registered with a Southampton GP, be over 16 years old and have a confirmed neurological diagnosis or have seen a Neurologist.
For patients registered with a New Forest, Test Valley and Southern Parishes GP, the service is available for 16 - 65 year olds with the exception of patients diagnosed with Motor Neurone Disease.

Portsmouth patients
Patients must be over 18 and registered with a Portsmouth City GP - with a diagnosis of Parkinson's Disease, Multiple Sclerosis, MND or TBI.
