Community Children's Asthma Service

The Community Children’s Asthma Nursing Service is available to children and young people with a Portsmouth City GP or a Southampton City GP

Our teams are made up of:

  • Portsmouth: one Community Children’s Asthma Nurse Specialist and one Community Children’s Asthma Nurse
  • Southampton: one Community Children’s Asthma Nurse Specialist

We provide specialist support and interventions to Children and Young People up to the age of 16 years old, with an asthma diagnosis or those with suspected asthma who are taking asthma treatment. 

 

About the service

The aim of the service is to improve the long term management of asthma in children and young people in order improve quality of life, improve asthma outcomes, subsequently helping to prevent and reduce hospital admissions.

We work closely with GPs, Practice Asthma Nurses, Consultant Paediatricians and Specialist Respiratory Teams.

 

What we do

  • Provide telephone, face to face, or virtual support as needed
  • Empower families to manage their child’s asthma at home
  • Provide a personalised asthma (inhaler) plan and safety netting advice 
  • Asthma education within current guidelines 
  • Reduce unnecessary hospital attendance and prevent hospital admissions

The referral route for this service is via health care professional only

  • School Nurse
  • Other Health Care Professional

Healthcare professionals can make an e-referral via SystmOne or by emailing or calling the team directly.

Information for Parents and Carers

Please contact your GP or Practice asthma nurse if:

  • Your child has not had an asthma review in the last year
  • Your child does not have an asthma action plan
  • Is needing their reliever inhaler more than 2 times a week
  • Is having frequent asthma symptoms affecting quality of life
  • Has had a recent hospital attendance or admission for asthma
