About the service
Cardiac Specialist Nurses support patients with heart disease and their family regarding diagnosis, treatment, recovery, rehabilitation, coronary risk factor management / secondary prevention / lifestyle changes.
What we do
We currently offer Cardiac Specialist Nurse reviews for patients registered with a Southampton City GP, GPSI Diagnostics via referral from a Southampton City GP, Cardiac Rehabilitation for Southampton City and Hampshire patients with qualifying conditions and Heart Failure reviews for Southampton City patients only, via GP referral or hospital.
Accessing the service
To access the service you must be registered with a Southampton City GP and be referred by your GP or hospital.
