The Community Rehabilitation Team is a multidisciplinary team that work in Springfield nursing home in Shanklin and the Community Unit at St Mary’s Hospital. They work collaboratively with primary care, hospital services, social services and the third sector.

The Community Rehabilitation Team is an NHS commissioned multidisciplinary team. We work in Springfield nursing home in Shanklin and the Community Unit at St Mary’s Hospital. We also work collaboratively with primary care, hospital services, social services and the third sector.
We aim to enhance the health, well-being and independence of adults living with physical and neurological deficits.
Our team consist of:
- Clinical Lead Nurse
- Advanced Clinical Practitioner (ACP)
- Occupational Therapists (OT)
- Senior rehab nurse
- Associate Practitioner (AP)
- Rehab assistant (RA)
- Social worker:
- Discharge coordinator:
Your daily care needs will be met by the care assistants who work in the home/unit.
What we do
The team aims to do a number of things:
- Help people to improve their function following accidents or ill health.
- Help people to manage their health conditions and maximize independence.
- Help people to make choices about their health and adjust to the impact it has on their life.
- Prevent unnecessary admissions to hospital.
- Provide specialist interventions for long term conditions.
Decisions about duration and intensity of input and support are made by the rehab clinicians involved and is based on individual need and the rehab goals set with you. This will be assessed once you are admitted to the placement.
Accessing the service
You must be referred into the service by a clinician or by your nursing home.
