Learning Disability - Intensive Support Teams 

There are two Learning Disability Intensive Support Teams. One team in Hampshire and the other Team in Portsmouth. 

About the service

The Intensive Support Team (IST) supports adults with a learning disability across Hampshire and Southampton who present with behaviours that challenge services. The team consists of Clinical Psychologists, Learning Disability Nurses, Certified UK Behaviour Analysts/Specialists, Speech and Language Therapists, Occupational Therapists, Therapy Assistants, Assistant Psychologists, PBS Practitioners and Administrators.

What we do

The IST are a tier 4 specialist service providing a responsive, structured pathway designed to offer focused support for service users and their carers within a defined period, supporting timely, person-centred care delivery. 

We offer an interdisciplinary team approach utilising a positive behaviour support framework. We aim to improve the quality of life of the service user and support staff teams and families to implement person-centred support strategies that are regularly reviewed and based on a Functional Behaviour Assessment, alongside other assessments the individual may benefit from. We offer training and upskilling to services and families to enhance implementation of these strategies. We liaise with external agencies to ensure a holistic approach to care.

Accessing the service

The service is open to people aged 18 and older with a learning disability, who engage in significant behaviours that challenge with a high level of risk. If the individual does not have capacity to consent to a referral, a decision can be made in their best interest. 

The IST consider referrals from the Community Learning Disability Teams or via the Single Point of Access.

The Intensive Support Team works with adults with a learning disability who are presenting complex, frequent and/or severe behaviours of concern, which requires an intensive approach to assessment and intervention. The team consists of a Behaviour Analyst, Learning Disability Nurse, Occupational Therapist and two Associate Practitioners.

We offer a multidisciplinary approach, utilising a positive behaviour support framework. We aim to improve the quality of life of the service user and support staff teams and families to implement person-centred strategies that are regularly reviewed and based on a functional behaviour assessment. We offer training and upskilling to services and families to enhance implementation of these strategies. We liaise with external agencies to ensure a holistic approach to care.

Accessing the service

The service is open to people aged 18 and older with a learning disability and significant behaviours that challenge, who meet our referral criteria and consent to seeing our team. If the individual does not have capacity to consent to a referral, a decision can be made in their best interests. We consider referrals from the Integrated Community Learning Disability Team, external professionals, Social Care settings, GPs, families and self-referrals. You can use this form to refer to the service.

The Intensive Support Team works 9am - 5pm Monday – Thursday and 9am – 4.30pm on a Friday. The team will aim to implement a flexible approach to working hours, should the assessment and intervention work require this.
