Rehab@Home

Rehab@Home -Supporting recovery at home across Southampton and South West Hampshire

Rehab@Home supports people who are ready to leave hospital to continue their rehabilitation and recovery in their own home, where this is clinically safe and appropriate.

Evidence shows that many people recover better at home, where they can remain independent, build confidence and continue their recovery in familiar surroundings.

Through Rehab@Home, patients can receive personalised rehabilitation and therapy support at home, helping them regain strength, mobility and independence while receiving the right care, in the right place, at the right time.

About the service

Rehab@Home provides rehabilitation and therapy support at home following a hospital stay. Rather than remaining in hospital for rehabilitation, suitable patients can receive their rehabilitation in their usual place of residence, supported by a team of healthcare professionals.

Patients receive personalised rehabilitation support for up to two weeks, helping them regain strength, mobility, confidence and independence.

Rehab@Home may be suitable for people who:

  • Are ready to leave hospital but still need rehabilitation support.
  • Need help to regain strength, mobility and confidence.
  • Would benefit from recovering at home rather than staying in hospital.
  • Have been assessed by their clinical team as safe to receive rehabilitation at home.

Recovering at home can help you:

  • Maintain your independence.
  • Regain confidence following illness or injury.
  • Improve mobility and everyday function.
  • Reduce the risk of losing strength and fitness during a prolonged hospital stay.
  • Continue your recovery in familiar surroundings.
  • Reduce disruption to family and daily life.
  • Receive personalised rehabilitation focused on your individual goals.

The enhanced Rehab@Home service is available across Southampton and South West Hampshire, including:

  • Lymington and New Milton
  • Totton and Waterside
  • Avon Valley
