The Neuro Early Supported Discharge Team (Neuro ESD) is a multidisciplinary team that treat patients who may have had an acute stroke or with other diverse neurological conditions across Southampton and parts of West Hampshire. The service is community based and comprises of two caseloads – the Community Stroke Team and Snowdon at Home however they operate as one team.

The Snowdon at Home service offers a specialist intensive rehabilitation to people over the age of 16 with a Neurological diagnosis following a recent neurological event or sudden deterioration in their condition.
Our team consists of Occupational Therapists, Physiotherapists, Nurses, Speech and Language Therapists and Rehabilitation Assistants.
We provide short term intensive rehabilitation and support patients in the community by working with them to achieve their individual rehab goals.
The service covers Southampton and some of the West Hampshire area.
Snowdon at Home Criteria:
- Registered with GP Surgery in Southampton City CCG and West Hants CCG within a reasonable commutable distance as Capacity allows (approx. 60 mins travel time from SO16 4XE)
- Present with a primary neurological condition
- Are an adult over 16 years old
- Would have previously received and benefited from intensive Neurological inpatient rehabilitation
- Will benefit from intensive ESD input
- Have consented (both client and carer) to community based rehabilitation
- Are medically stable
- Have rehabilitation goals that can be achieved in home environment/community
- Are safe overnight (8pm-8am)
- Have a safe home environment for client and service providers.
- Are transferring with a maximum assistance of 1 where there is no care plan/POC in place

Referral into this service is via a healthcare professional referral only. If you feel your patient meets the above criteria and will benefit from short term intensive rehabilitation, please get in touch with us to discuss further and complete the appropriate referral form. Referrals must be discussed and agreed prior to the patient’s discharge home from hospital.
