About the service
Our Community Neurological Service specialists in the treatment and rehabilitation of adults with neurological conditions such as Multiple Sclerosis, Parkinson's Disease, Motor Neurone Disease, Head Injury, Cerebral Palsy and Stroke.
Services are provided in Portsmouth and Southampton, working alongside existing health and social care services to provide coordinated, patient-centred care.
The teams consist of:

- Specialist Neuro Occupational Therapists
- Specialist Neuro Physiotherapists
- Specialist Neuro Speech & Language Therapists

- Doctors, Dietitians, and Rehabilitation Assistants
The team also works closely with the Community Neurological Rehabilitation Gym at Adelaide Health Centre in Southampton, offering individual neurological assessments, treatment sessions, and self-management support.

Once referred and accepted, patients remain under review and can self-refer back into the service as needed.
Support will be provided to health professionals in other services to improve the quality of care for patients with neurological conditions.
Accessing the service
ll referrals to the service will be triaged by a clinician. Patients accepted to the service will be sent a letter to confirm they are on the waiting list for an assessment. Referrals with insufficient clinical information will be returned to referrer.

To be referred by a health professional to the Southampton team you must be:
- Registered with a Southampton GP
- Be over 16 years old
- Have a confirmed neurological diagnosis or have seen a Neurologist.
Please note. For patients registered with a New Forest, Test Valley and Southern Parishes GP, the service is available for 16 - 65 year olds with the exception of patients diagnosed with Motor Neurone Disease.
About the service
This services provides individual neurological assessment and treatment sessions, as well as providing self management for people living with long term neurological conditions.
Made up of Physiotherapists with a special interest in neurology and Rehabilitation Assistants, we offer specialist clinics for Functional Electrical Stimulation, Orthotic clinic, Spasticity clinic, and this sees us working alongside other healthcare professionals to provide co-ordinated and effective rehabilitation.

About the service
Our Community Neurological Service based in Portsmouth is a Multidisciplinary Team that supports adults [+18] with a confirmed diagnosis of Multiple Sclerosis, Parkinson's Disease, Motor Neurone Disease and Traumatic Brain Injury.
The service consists of:
- Nurse Specialists in Parkinson’s Disease and Multiple Sclerosis,
- Clinical Psychologist in Neuropsychology
- Specialist Neuro Occupational Therapists,
- Specialist Neuro Physiotherapists,
- Specialist Neuro Speech & language therapists,
- Associate Practitioners
- Rehabilitation technicians
Specialist Occupational Therapy and Physiotherapy also provide specialist neurological assessments and treatment to adults with any confirmed neurological diagnosis.
The service aims to support:
- Providing access to a range of co-ordinated, multi professional services and interventions that are patient centered to enable patients to retain optimal independence within their chosen environment.
- Support with self-management and self-empowerment of their condition, including education, goal setting, care planning, problem solving, lifestyle advice and support.
- Symptom management as conditions progress
- Provision of evidence-based interventions and specialist equipment if clinically indicated.
- Offering regular review, monitoring and self-referral access when required.
- Signposting to other services as needed.
- Supporting professionals providing ‘usual services’ to improve the quality of care across community services for patients with neurological conditions.
Once known to the team and have an open referral they will remain on review and are able to self-refer to disciplines when required.
Who can be referred to the service?
- Adults over 18 years of age registered with a Portsmouth City GP and a confirmed diagnosis of Parkinson's Disease, Multiple Sclerosis, MND or TBI.
- Occupational Therapy and Physiotherapy referrals will be considered for patients with other neurological conditions.
About the service
The Neuro Gym Physiotherapy Outpatients service offers neurological assessment and treatment sessions for adults (aged 18+) with a confirmed neurological condition within an outpatient setting.
The service consists of:
- Specialist Neuro Physiotherapists
- Rehabilitation technicians
Patients receive an assessment of their problems related to their neurological condition and how it impacts on their function. The team will recommend which treatments are most appropriate and agree patient centred goals.
A primary aim is to support people living with a long-term neurological condition to develop the skills to feel confident to self-manage their condition.
Treatment interventions may include:
- Exercise therapy
- Development of home or gym-based exercise programmes.
- Supervised gym class sessions (defined number of sessions)
- Gait (walking) re-education and assessment of walking aids.
- Moving and handling guidance
- Long term self-management advice and support
- Referral to local exercise referral scheme.
Some additional assessments offered are:
- Electrical stimulation for upper and lower limb (where indicated as part of a rehabilitation programme).
- Spasticity assessment and management (in liaison with consultants where indicated)
- Assessment for specialist equipment/ devices as part of a rehabilitation programme and onward referrals to other services where appropriate. For example, referral to orthotic department for consideration of orthotic splints.
The service provides an episode of goal-orientated rehabilitation following which patients are discharged with self-management plans.
The service is based at Queen Alexandra Hospital in Portsmouth. The rehabilitation goals must be able to meet in an outpatient setting.
Who can be referred to the service?
The service is available to adults over 18 years of age who have a confirmed diagnosis of a neurological condition.
The patient must be registered with a GP within the Portsmouth and South-Eastern Hampshire area. Please contact the team to confirm eligibility.
The primary problem for rehabilitation under this service must relate to their neurological condition and an outpatient setting the best environment for the treatment to occur to meet the patient’s needs.
How to refer
Referrals to the team are made by a health care professional. A service referral form is available on request.

The referral should include at least the following information to allow triage:
- Reason for referral / problem identified.
- Relevant history
- Current functional status

What happens if I am the patient and referred to the team?
All referrals to the team are reviewed by a physiotherapist. We will write to you to let you know we have received your referral, and you are on the waiting list.
We will offer you an appointment for an initial assessment. This appointment date and time will be made via phone and / or letter with confirmation via mobile text messages.
If the appointment offered is not convenient, please call our team on 02392 286509 or 0300 123 5009 to arrange an alternative appointment.
What to do on the day of your appointment?
The Neuro Gym Physiotherapy Department is located on Level C in the REHABILIATION CENTRE at Queen Alexandra Hospital, Portsmouth, PO6 3LY.
On arrival to the Rehabilitation Centre, please report to the hydrotherapy reception/ waiting area on level C. Your physiotherapist will meet you from the waiting area.
Details regarding parking at the hospital site will be provided to you at time of booking an appointment.
