Community geriatrics - Southampton and South West

We are a team of geriatricians working in the community, in various teams and settings to support individuals living with frailty (aged 65 and over).

Accessing the service

Our clinicians each cover a locality for domiciliary visits based around one or more Primary Care Networks. We are also happy to provide advice.

Typical referrals for visits include:

  • Complex comorbidities, falls, reduced mobility and frailty
  • Complex polypharmacy and deprescribing
  • Complex medical and social problems including cognition and capacity
  • Frequent presentations to primary and secondary care
