We are a team of geriatricians working in the community, in various teams and settings to support individuals living with frailty (aged 65 and over).
About the service
Our main clinical roles include:
- Performing domiciliary visits (comprehensive geriatric assessments) for housebound individuals or those with significant packages of care
- Outpatient clinics at Lymington New Forest Hospital
- Parkinson’s outpatient clinics and domiciliary visits
- Inpatient rehabilitation at the Western Community Hospital, Romsey Hospitaland Lymington New Forest Hospital
- Supporting Urgent Community Response/Hospital at Home services
- Supporting community nursing and rehabilitation teams
- Acute general and geriatric medicine at both University Hospital Southampton and Lymington New Forest Hospital
Accessing the service
Our clinicians each cover a locality for domiciliary visits based around one or more Primary Care Networks. We are also happy to provide advice.
Typical referrals for visits include:
- Complex comorbidities, falls, reduced mobility and frailty
- Complex polypharmacy and deprescribing
- Complex medical and social problems including cognition and capacity
- Frequent presentations to primary and secondary care
