Our nurses work across community hospitals, inpatient services, community services and community mental health services, delivering care in people’s homes, local settings and specialist environments. Working as part of multi-disciplinary teams, they provide coordinated, person-centred care that supports recovery, promotes independence and improves outcomes for communities across Hampshire and the Isle of Wight.
Physical Health Nursing
Our Physical Health Nursing teams work across hospitals, clinics and people’s homes, offering a diverse range of opportunities across community and inpatient settings, supporting people at every stage of their health journey:

Community hospitals: inpatient rehabilitation, including stroke and neurological rehab, step-down care from acute hospitals and end of life care

Community and clinic-based services: wound care and leg ulcer clinics, bladder and bowel services, heart failure and cardiac reviews, diabetes support, chronic pain services, urgent community response

0-19: children and family services, health visiting, school nursing

Specialist support: neurological rehabilitation, dementia services, endoscopy, special care dental and sexual health services

Primary and outreach care: primary care services and dedicated homeless healthcare provision.

Mental Health Nursing
Our Mental Health Nursing services span community and inpatient settings, offering a diverse range of opportunities to support people at every stage of their mental health journey, delivering coordinated, person-centred care that supports recovery, promotes independence and improves quality of life:

Inpatient services: Forensic services, Psychiatric Intensive Care Unit (PICU), Eating Disorders, CAMHS, Mother and Baby Unit (MBU) and Older Persons Mental Health services

Community mental health services: ongoing support for adults, children and young people, including specialist pathways and recovery-focused care

Crisis services: crisis resolution and home treatment teams providing urgent assessment and care to help people stay safely at home where possible

Early intervention: services such as Early Intervention in Psychosis, supporting people at the earliest stages of mental illness

Specialist services: including dementia care, perinatal mental health and other targeted support pathways

Learning Disabilities Nursing
Our Learning Disability Nursing services span community and inpatient settings, offering a range of opportunities to support people with learning disabilities through coordinated, person-centred care that reduces inequalities and improves access to services.

Community Learning Disability Teams: supporting people across Hampshire, Southampton, Portsmouth and the Isle of Wight to understand and manage their health needs

Inpatient services: including Learning Disability forensic mental health services, supporting individuals with complex needs in specialist settings

Multi-disciplinary working: collaborating with occupational therapists, speech and language therapists, psychologists, psychiatrists and physiotherapists to deliver coordinated care

Health facilitation and liaison: supporting access to mainstream services such as GP practices and hospitals, and ensuring reasonable adjustments are in place
-
Specialist support: working with people with a range of needs, including mental health, epilepsy, dementia, complex physical health conditions and behaviours that challenge
