Early Supported Discharge

About the service

Part of our Community Neuro Rehab Service, the Early Supported Discharge (ESD) team helps patients after a stroke or neuro event to continue their recovery at home, with the same level of specialist rehabilitation they would receive in hospital.

An intensive six week pathway, the team supports patients who are clinically suitable for ESD, ensuring a safe transition home while focusing on quickly regaining independence and achieving meaningful goals in everyday life.

It’s a personalised, multidisciplinary service designed to improve outcomes, reduce hospital stay and support people to rebuild their lives after stroke.

Working in the service

The team supports patients at a key stage of recovery, helping them transition from hospital to home while continuing structured rehabilitation programmes tailored to their needs.

The work is varied and goal-focused. One day you might be helping someone regain mobility and confidence at home; the next, supporting communication, daily activities or emotional adjustment following a neuro event.

Care is coordinated closely with hospital stroke teams, patients and their families, ensuring continuity and a seamless experience across the whole pathway.

Roles in the service

The service is delivered by a multidisciplinary team bringing together a range of specialist skills to provide high-quality neuro rehabilitation in the community.

This typically includes:

• Physiotherapists supporting mobility, strength and balance
• Occupational Therapists focusing on daily living skills and independence
• Speech and Language Therapists supporting communication and swallowing
• Nurses providing clinical care and ongoing health support
• Psychologists supporting emotional wellbeing and adjustment
• Rehabilitation Assistants supporting therapy programmes
